The shift to value-based care requires implementing new models of care, new financing models, and managing entire populations instead of individual patients. This is especially challenging in the safety-net environment where resources are already limited.

This 12 module series provides a basic understanding of population concepts and tools – and how this differs from clinically focused work. It is designed to help increase clinician knowledge about population health and build the skills needed to navigate this new territory. Modules can be completed in any order.
